To submit a Creator profile or to update an existing one, please use the form below.

The following information is required to submit a Creator Profile:

▪ Creator Name
▪ Profile Type – see table below
▪ Creator Location *
▪ External Webpage – one URL
▪ Creator Disciplines – view disciplines
▪ Short Description – up to 100 words (optional)
▪ Profile Image – one image at max 1350×900 72dpi (optional)

* To be a part of this map, each Artist / Creator / Group must be linked to a Space.
If your space is not listed in the EAD Map/Directory, submit Space first along with your Artist / Creator / Group profile.
